Not really specific to Debian distros:
Updating VBox is fairly easy thanks to the apt repository. Unfortunately the extpack has to be manually downloaded and installed every time. This could be solved by packaging the extpack. Users could then choose to "apt-get install virtualbox-extpack" or something similar, and not have to mess about with each new release.
